will i have to get a root canal or is it just a cavatie?

i have a few cavities and none of them really hurt until recently. i was waiting to get them done until my insurance got renewed but this particular cavitie is getting worse by the day. it''s my molar furthest back on the right side on top. its got a hole near the gum, my actual tooth has a hole. what does this mean? will i have to get a root canal or is it just the cavatie itself that needs to be filled? i''m really nervous and don''t have a lot of money to pay for such treatment without insurance.
